your last check date with the Tax Service is your last check date of the quarter and the Tax Service is to file quarterly returns. Termination date is the last day of the quarter. |
Specify if the FEIN being terminated is ceasing operation and if the Federal 941 return should be filed as "Final." When this is the case, the termination date is the last day of the quarter.
If the Tax Service is filing a "Final" 941 return during first, second, or third quarter, specify if you want the Tax Service to file accelerated W-2's and annual returns. If your business is being dissolved, include the name, address, and telephone number that should be used by the Tax Service after the business is dissolved. When preparing quarterly or annual returns for a dissolved business, the Tax Service must be able to contact a company representative who will be available to address any problems or to provide additional funding, if necessary.
You may be required to notify individual agencies to advise them if you cease operations.
2. Forward your termination letter to your Tax Service Representative.
Provide your Tax Service Representative with your signed letter at least thirty (30) days prior to your termination date. Your notification will be forwarded to our Client Closure department for handling. If you have any questions regarding the closing of your account, please contact your Tax Service Representative.
