When you receive the QE Planner for 2Q and 4Q, you will receive your Master File Audit Report for each division within one client ID; i.e., FEIN Federal Employer Identification Number. This is the same report that you received in the Welcome Package.
The Master File Audit Report is a summary of the data currently found in your Client Master File. It determines how the Tax Service accesses funds and where we make deposits and file returns. This information is also used to make deposits at the proper rates and intervals and to identify your company on returns and wage listings.
When you receive your Master File Audit Report, complete the following steps to ensure all of your company information is correct. Refer to the Start-Up chapter for detailed information on Reviewing the Master File Audit Report.
1. Review and confirm the information in the following data fields:
• Mailing address
• Client bank information
• Legal name and address
• Client contact, phone, fax numbers
• Funding method
• Client tax code information, including deposit frequency, SUI State Unemployment Insurance rates, and EFT Electronic Funds Transfer access codes
Pay special attention to any tax code listed on the report with "AF Applied For" in the Tax ID field. This is an indication that we have not yet received from you your agency-assigned identification number (ID).
Many jurisdictions do not accept filings without an assigned ID; therefore, it is very important that we receive the ID, or at least a copy of the completed agency registration form, before quarter-end filing begins.
2. Make any necessary changes to the report.
If any information is incorrect, cross through the information and write in the correct information or contact your Tax Service Representative.
Changes to your bank information or legal name and address require additional documents. Refer to Changing the Bank Account Used to Fund Liabilities or Changing Your Company's Legal Name sections of the Maintenance chapter for details.
3. Sign the report, attach any supporting documents, and forward them to your Tax Service Representative
Attach supporting documentation, such as agency documents, for any changes made to the data and sign the report. If you did not make any changes, you are still required to sign the report acknowledging that all the data is correct. Send the updated report to your Tax Service Representative by the deadline published in the QE Planner.
