Standard State Power of Attorney

This form grants the Tax Service authority to file state taxes on your behalf in those states that require a separate power of attorney rather than the Tax Information Authorization. Most states will accept a photocopy of our Standard State Power of Attorney that has been signed by a representative of your company. For these states, the Tax Service can retain a copy of this signed generic form for future use. A few states do require a Standard State Power of Attorney with an original and/or notarized signature.

1. Name/Address: Enter your company name and address, and check the type of your organization.

2. State: Enter the name of the state to which this power of attorney applies. Do not abbreviate.

3. Applicable Tax: Check the type of tax for which this power of attorney is being granted.

4. State ID Number: Enter the state tax ID number for the tax checked.

5. Employer-Taxpayer: Enter the company name as entered in the Name/Address data field.

6. Signature: Sign and date the form.
